By submitting information via contact form on the home page, the message is redirected from form-submission@squarespace.info to therapy@jeanineclaus.ca.  The information collected in the contact form is limited to the information provided by user, for the purposes of answering inquiries related to services provided or booking free consultations with practitioner to determine if services are appropriate for user.  Because of the nature of e-mail transmission and potential for malicious third-party activity outside of jeanineclaus.ca or Squarespace, user is responsible for insuring that personal health information, financial information or other types of information not necessary for the booking of services or answering general inquiries are not included in the form submission.  This website will not ask for Personal Health Information but may be collected at a later date after receiving consent from the client, which is protected by another Ontario provincial law, the Personal Health Information Protection Act, 2004, S.O. 2004, c. 3, Schedule A (available at https://www.ontario.ca/laws/statute/04p03 ).
